ATHLETE APPEARANCE REQUEST
Request a National Women’s Team Athlete

If you are you having an event and would like a National Women’s Team member to appear please fill out the Athlete Appearance Request Form – explain what your event is, when it is and where it is. Hockey Canada will forward the request to the athlete (s) requested.  It will be up to the athlete (s) requested to reply directly to your request. Directly.  

Requests should be received at least 30 days in advance of the event to leave enough time for distribution to the requested athletes. 

Submission of this form does not guarantee that an athlete will be attend. Each individual athlete will decide which event (s) they are available to attend.
